Upload all required documents, completed and signed:
- Registration form.
- Valid medical certificate.
- U18 consent form for athletes under 18.
- U14 consent form for leaving unaccompanied, only for children under 14 to whom this applies.
The U14 and U18 forms apply only to their respective age groups; for children under 14, check the U18 form too. Give the original medical certificate to your coach or team manager.

First-time Under 15 registration is always a new membership and includes the team kit. For families with at least one child who is a member, the rules provide a 20% discount on fees below the highest fee (or on one of two equal fees).

READY TO TAKE THE FIELD
Medical certificate.
To take part, you need a valid medical certificate appropriate for Flying Disc and your type of activity. Before booking, ask your coach or team manager for the request form and guidance for your category, and take these to the sports medicine centre. The doctor determines the applicable protocol and tests: we do not specify the same A1 or B1 code for everyone. Upload a copy and give the original to your coach or team manager.
Under 18s
The certificate is free when issued by a medical centre accredited with Regione Lombardia. A request form from the club is required; collect it from your coach during training.
- Book a competitive sports medical examination for the discipline officially named Giochi e Sport Tradizionali – Disciplina Flying Disc
- Give the club’s request form to the doctor, who will determine the protocol and tests for the discipline and age category. If you are unsure about the wording of the certificate, contact your team manager before the appointment.
Check appointment availability and the required documents with the centre.

There is a charge for the certificate, and no request form from the club is required.
- Book a competitive sports medical examination for a certificate on plain paper/for private use, for the sportFlying Disc.
The certificate must state the name of our sport. - Arrange with the centre for a certificate covering Flying Disc and the type of activity specified by the club. The doctor determines the protocol and tests.
Not all medical centres issue this type of certificate.
Below are the centres listed by the club: confirm the procedure, availability and prices directly with each centre.
Leonardo Medical
2026–2027 agreement, valid until 31 July 2027.
Non-competitive sports examination: €40 with resting ECG; €55 with resting and post-exercise ECG; €65 including spirometry.
Competitive sports examination (B1), using the age bands stated in the agreement:
- Men under 40 / women under 50: €70.
- Men over 40: €90.
- Men and women over 50: €110.
The €90 and €110 examinations require recent blood tests including total cholesterol, HDL and blood glucose. Confirm the applicable age band with the centre.
10% off ultrasound, cardiology, orthopaedics, podiatry and other services. Proof of current membership with an active subscription is required to access the agreed rates.
